PHAR 766 - Transitions of Care This elective will assist students in an understanding about a pharmacist role in transitions of care (TOC) to decrease 30-day hospital readmission rates. It will explore pharmacy’s role in TOC for various settings including community pharmacy, ambulatory care, hospital pharmacy, subacute rehabilitation, and special populations. This will be accomplished by exposure to lecture, discussion, interprofessional simulation, as well as direct patient care in the hospital and ambulatory clinic settings. All lectures will be completed by pharmacist participating in TOC services. Pre-requisite Course: PHAR 624 with a D- or better. Term Offered: Spring Level: GR Credit Hours: 2
